Registration: If your child is going into Kindergarten or new to CPS for the school year 2024-2025, you will need to bring the following documents:
**2 proofs of current residency (utility bill, lease or mortgage, voter registration, auto/insurance registration)
**proof of identification (driver's license or state ID)
**child's original birth certificate or passport (no copies)
**child's up-to-date physical/immunization records/ allergies or other health conditions
**child's up-to-date dental information
**child's eye exam information (kindergarten only)
**2 pay stubs (PreK only)
All medical, dental and eye exam information must be current for the 2024-2025 school year.
All medical and dental information must have dates past September 1st, 2023 until more current documentation can be shared. For example. if you have medical documentation dated September 5, 2023, and have an August appointment, you can still register. You will share the September documentation and then once the August appointment has occurred, you will give a copy to the office to update the information and avoid future out-of-compliance correspondence.
If your child is going into 1st, 2nd, 3rd, 4th, 5th, 6th, 7th or 8th grade for the school year 2024-2025 AND they are already CPS students, you will need to bring the following documents:
*2 proofs of current residency (utility bill, phone bill, lease or mortgage, voter registration)
*updated physical/immunization records/ allergies or other health conditions and dental information
**6th graders need updated physical and immunization records
*proof of identification (driver's license or state ID)
*inform your current school of your plans to transfer so they can "project" the student to NTA.
This year we are not accepting medical appointments prior to registration. All documents need to be brought in at the time of registration. We are not able to make copies or print documents from emails.
